1.  Explain the creation of Learning Center in detail (matching the Grade Level of the lesson plan it is connected to).  Include ALL materials to put the center together and where they can be purchased  (4pts)

2.  Include the Grade Level, Standard, and Objective of the Learning Center (these must match your lesson plan)  (4 pts)

3.  Explain how your Learning Center represents your Lesson Plan (what is it they learned in the lesson plan and how it connects to what they are practicing in the Learning Center).  4 pts.

4.  You MUST have an Accountability Sheet  (Recording Sheet)  and explain how it is completed by the students  (this is so you can assess that they did their work in the center and if they are doing it correctly (4pts)
