– A discussion of the short and long-term physical health risks arising from your client’s physical health assessment (details will be provided separately) (c.600 words)
– An exploration of the interventions planned to promote physical care for this client (c. 1000 words) – chose two – three (max) interventions.
– A referral letter to a relevant agency or practitioner (c. 200 words)
