My healthy people 2020 goal is to increase the proportion of children aged 0 to 11 years with special health care needs who receive their care in family-centered, comprehensive, and coordinated systems (Office of Disease Prevention and Health Promotion [ODPHP], 2017). Three websites that correlate with this goal are the Centers for Disease Control and Prevention, Early Childhood Development, and The Arc.
The Centers for Disease Control and Prevention is an example of primary prevention because it provides information for people with disabilities about healthy living, safety, school, transitions, independent living and finding support. The goal is to help people with disabilities become more independent and involved in the world (Early Childhood development, 2017).
Early Childhood Development promotes a joint federal approach to improve early childhood education and development (“Early Childhood Development,” 2017). This website is an example of secondary prevention, it recognizes that developmental and behavioral screenings track a child’s progress in areas such as language, social, or motor development (“Early Childhood Development,” 2017).  It provides screening tools to families, care providers, and education providers in order to increase the amount of children properly diagnosed at an early age.  They believe that early identification allows communities to intervene earlier, leading to more effective and cheaper treatment for children during their preschool years (“Early Childhood Development,” 2017).
The Arc is the nation’s leading advocate for all people with intellectual and developmental disabilities and their families (“The Arc,” 2016).  Their website provides information and referral services, individual advocacy to address education, employment, health care and other concerns, self-advocacy initiatives, residential support, family support, employment programs, and leisure and recreational programs (“The Arc,” 2016).  This website is an example of tertiary prevention because it provides support and therapies for those with disabilities and their families in order to enrich their lives.  Their goal is to improve people with disabilities level of functioning.
Screening is a valuable tool in maintaining the health of people in the community because screening can help detect disease and other illnesses as well as modifiable risk factors.  According to Maurer and Smith (2013), “An essential component in maintaining the health of the community is early detection of disease.”  (p. 486).  Health promotion focuses on changing patterns of behavior or environmental structures to promote health (Maurer & Smith, 2013).  By screening individuals and communities one can detect potential risk factors that contribute to poor health.  Education can then be done to help people change these modifiable risk factors.  Without screening one may not know they are at risk for certain diseases.  Screening programs are either targeted toward individuals or to populations (Maurer & Smith, 2013).  Screening test for individuals are decided based on the person’s history and risk factors (Maurer & Smith, 2013).    A screening program for the population is based on general sociodemographic (Maurer & Smith, 2013).  The nurse must take into consideration the age, gender, and ethnicity factors of the target population and base the screening off of the population as a whole (Maurer & Smith, 2013).

Stroke is an injury to the brain that happens suddenly and without warning a lot of the times. Anyone that is 18 years of age or older is at risk and the risk is multiplied with certain modifiable and non-modifiable risk factors. With screening patients and people of the community, we can identify these risks earlier and help to reverse some of them before it is too late. This website does a great job of identifying these risk factors. They are categorized into three areas; uncontrollable risk factors, medical risk factors and lifestyle risk factors. By screening patients for these types of risks and treating them early, we are helping to improve the health of the community. While preventing stroke you are concurrently treating cardiovascular disease and in turn reducing the number of heart attacks as well.

Once a stroke injury has occurred it is essential to know the best way to treat the stroke. By figuring out why the stroke occurred can usually help to prevent a recurrence of a stroke. This website does an excellent job of discussing how a stroke is diagnosed, which labs are drawn, which tests are ordered, which procedures are performed and which medications are ordered as well. It discusses the different areas of stroke in the brain and how each area results in different symptoms.

A lot of the time, strokes are debilitating. They cause catastrophic damage to the brain and to the body as well. Recovering from something as devastating as a stroke takes a lot of encouragement, motivation, rehabilitation, more education, practice, and most importantly, time. The modern medicine website provides support in most of these departments. It lists resources for the stroke survivor as well as stroke survivor stories. It provides links to more stroke websites for additional information. It teaches what things to look out for and what obstacles the patient still might need to overcome. It guides the patient in medication options, and surgery options (if applicable). It also provides research trends which I think is a very cool feature.
