The following directions are to be used to develop a Project Outline for a final report in the course. Please be sure to review all items and the following example to assist in developing your outline. 

Step 1: Choose an Organization

Step 2: Go to their website and learn about them. Use bullet-points for information that you find 

(be sure to list their website)

Step 3: Google search them (Must have a minimum of three articles and two social pages)

** This is a minimum for the outline requirement. You should have more sources for your organization (especially articles) within your final report. 

Step 4: Write the information you have learned from the organization based on what you found through news sources and social media?

**This needs to be a minimum of five pieces of information per article and social page. Therefore, you should have a minimum of 25 bullet points of information to meet this assignment requirement.

Step 5: Find your competitorlist three reasons why they are a good competitor based on your organization’s knowledge. Use a minimum of two sources to support your reasoning. 

Step 6: Determine at least one strength, one weakness, one opportunity, and one threat to the organization. This should come from website information, articles, and social information collected.

*Reminder: Strengths and Weaknesses use information from the company. Opportunities and threats use information that you collected from your competitor or . 

Step 7: Brainstorm three ideas to address an area of the SWOT (Strength, Weakness, Opportunity, Threat)

Weakness: Brand recognition (Organization’s true name vs. flagship product), weak engagement on social channels

Opportunities: specialized items (like knifeless knife, oyster knife), re-focusing on North American subsidiary (largest market), can rebrand to younger audiences looking for custom items and niche tools, communicate more personal connections with the brand through social media.

Threat: Areas of growth present with numerous players in the market, may make it difficult for the company to differentiate itself and their products, brand tag is not universal across all platforms

Step 7: Brainstorm three ideas to address an area of the SWOT (Strength, Weakness, Opportunity, Threat)

1.) SWOT: THREAT (BRAND TAG NOT UNIVERSAL) 

Determine what the social tag will be, and update across all branded platforms

b. Minimize threat issues

2.) SWOT: STRENGTH (INCREASE EMPLOYEE EFFECTIVENESS)

Create a benefit for employees, further strengthen the good will and lack of sick time taken by employees

a. Provide questionnaire to employees asking about desire for using some sick days as mental health days

b. Evaluation: Year-over-Year (YOY) assessment of company moral, sick days used, compared with company effectiveness

Assessment 3 Instructions: Evidence-Based Population Health Improvement Plan

Create a 3-5 page work identifying the health concern that you think is most appropriate to address for the community in your practice environment. Your choice should be based on the evaluation the relevant data that you have gathered for your chosen issue. 

Scenario

Your organization has created an initiative to improve one of the pervasive and chronic health concerns in the community. Some examples of possibilities for health improvement initiatives are nationwide concerns, such as type 2 diabetes, HIV, obesity, and the Zika virus. However, your organization wants you to identify the health concern that is most appropriate to address for the community in your practice environment. You will need to do your own research to gather and evaluate the relevant data for your chosen issue.

The bullet points below correspond to grading criteria in the scoring guide. Be sure that your population health improvement plan addresses all of the bullets below, at minimum. 

Evaluate the environmental and epidemiological data about your community so that you can illustrate and diagnose widespread population health issues.

Develop an ethical health improvement plan that addresses the population health issue you have identified in your evaluation. The plan should be based upon the best available evidence and meet the cultural and environmental needs of your community.

Justify the value and relevance of the evidence you used as the basis for your population health improvement plan.

Propose criteria that can be used to evaluate the achievement of your health improvement plan’s outcomes.

Explain how you plan to apply strategies for communicating with community members and colleagues in the health care profession in an ethical, culturally sensitive, and inclusive way about the development and implementation of your health improvement plan.

Integrate relevant sources to support assertions, correctly formatting citations and references using APA style.

Length of submission: 35 double-spaced, typed pages. Your plan should be succinct yet substantive.

Number of references: Cite a minimum of 35 sources of scholarly or professional evidence that support your evaluation, recommendations, and plans.

APA formatting: Resources and citations are formatted according to APA style.
