Read chapter 5 of the textbook, paying special attention to the case study of James Trent. You are going to be following the same instructions as in the model used for James Trent.Once you have read through chapter 5, read through the case study used for this assignment (case 16.6 Stress and Substance Abuse from the DSM-5 Clinical Cases book). As you read through the case study, highlight or circle every piece of information that can be considered abnormal, out of the range, or significant as illustrated on pages 70-71 of the text. If you are not able to print out the case, you may want to make a running list.Once you have highlighted the relevant issues, complete Table 1 on page 2. Review the model on page 72 of the text for assistance. You are to identify 5 aspects of the clinical presentation that are related to substance use (left hand column of the Table 1), and 5 aspects of the clinical presentation related to mental health (right hand column of Table 1).Once you have completed Table 1 you are ready to proceed to Table 2 on medical issues. As discussed in the text, medical issues are an important aspect of the clinical assessment. In Table 2 on page 2, indicate what, if any, medical conditions (including allergies) the client has. Important: Never leave a space blank. If it was not documented, it can be assumed the question was not asked, and this can have important implications from a clinical and a legal perspective.This pertains to all aspects of treatment and is especially critical involving documentation related to diagnoses, risk assessments, and medical issues. If there are no known medical issues or allergies, then type in “No known medical issues or allergies.” This implies that you explored whether there are medical issues. If you leave the space blank, it is not clear if anyone asked about medical issues. 5.Once you have completed Table 1 and Table 2, you are going to create a Problem/Need List similar to the one illustrated on page 73 of the text. The goal, as stated in the text, “is to distill the material into succinct statements around which you and your client will construct treatment.” Keep in mind that the order listed should present the most serious and pressing concern first. In Table 3 on page 2, write 1-3 sentences about the 3 most serious concerns you identified in tables 1 and 2. At least one response should address substance use, and one response should address mental health issues. Refer to page 73 of the text for an example of how to write a concise Problem/Need statement.
