The chief complaint is “chest pain.” The history of present illness includes the patient’s description of the chest pain, including its onset, location, duration, intensity, and associated symptoms. The past medical history includes the patient’s history of heart disease, hypertension, and diabetes.
Additional information that should be included in the documentation includes the patient’s family history of heart disease, their smoking and drinking habits, and their current medications. The patient’s weight and height should also be documented.
