In this assessment task, you are expected to evaluate a current professional gerontological practice model in Australia as identified in Module 2 and how it may enable and support the decision making of older clients. As a potential leader in the field of gerontology and aged care, you will examine the practice model within the context of your existing service/practice in either primary, residential, sub-acute or acute care.  An example may be transition, integrated GEM, evidence-based care, relationship centred and/or integrated or pathways model of care such as short or long-term restorative care or rehabilitation in sub-acute care. It may be applied to any practice setting.

In your critical analysis of the model synthesise both policy and evidence-based practice frameworks with the view to:

identifying challenges in service design and
practice that impact on your ability to lead and manage in the service in the health work.
