1. Explain at least 3 benefits and 3 challenges that you have identified of each (primary care and chronic care) model of health care.

2. Think about which you think is the better model and explain why you have the opinion you do.

3. Next, suppose your organization’s CEO wants to adopt an organizational model of care and has requested persuasive presentations from the staff. Write an outline that you could use to give a 2-minute presentation supporting your model of choice. Write in an outline style. Note: Do not write introduction & conclusion.
