Knowledge, skills and attitudes: We want to understand what the providers understand about prostate specific antigen (PSA) testing, what skills they feel they have in shared decision making, and what their attitude is with both PSA testing and shared decision making.

Usefulness of the CDSS system: We want to assess how useful CDSS is in making clinical decisions

Adoption of the CDSS tool: We wanted to assess how much providers are using the CDSS tool.

Usefulness of the educational training: We wanted to assess attitudes towards the PSA-specific educational training.

Utilization of the PSA test: We want to assess how frequently and in what populations clinicians order PSA testing

Further test with 5 “public health professionals” to assess the estimated time required to complete the survey tool.
