Accident Investigation for the Root cause
Introduction
The word accident is usually regarded as the unplanned event that usually interrupts the end of a process or an activity. An accident may include injuries and the damage of properties. It may be defined as an unforeseen event that did not inflict damages or injuries, but had the potential to do so. The term accident and incident may be interchangeably used. However, accident is argued to refer to any event that is related to both fate and chance. There are various reasons as to why an accident should be investigated. The reasons are as follows; to fulfil some of the lawful requirements, to establish the actual cost of the accident, to process the workers’ compensation claims, to find out the various causes of the accident and so on. The accidents that entail no injuries or damages should be investigated as well to determine the causes that should be corrected.

Background
Over the years there have been many accidents of various kinds that have emerged. The accidents may have occurred with no ill intentions. However, majority of the accidents that have been in constant occurrence are usually as a result of anger release, or with intentions aimed at causing intentional harm to a person. As a result, accidents should be well investigated from its root level to get the motive of the cause of the damage or injury. The investigation of an accident should ideally be conducted by someone who is well experienced and has full knowledge of the processes and the correct procedures that should be used. In most cases, the safety officer, a health and safety committee, employees that have experience in investigations should conduct the investigations.

Body
Major points
Accident investigation is a critical part of control of various losses. When correctly done it enhances safety as well as reduce costs. The damage of property as a result of accidents can cost a company a lot more than the compensation the law requires for the workers.
A lot of accidents in organizations are more in industries with very high amounts of the invested capital. Such organizations should place some amount aside that will aid in the maintenance of the equipment to avoid accidents that may result in loss of lives and an added cost.
One of the best ways of conducting a good investigation on the root of the accident is by investigating even the minor accidents that occur (Strauch, 2015). This will help in reducing the possible major accidents that have the potential of occurring. However, the more the number of accidents reported, the more the problems investigated to avoid major occurrences that will result in the loss of many lives.

Minor points
In a company, a safety manager is responsible for accident investigation. With limited resources, however, a safety manager cannot conduct proper investigations on the reported accidents. Therefore, resources should be set aside for the safety manager to conduct ample and satisfactory investigation on a reported accident.
A safety manager should not conduct direct investigation unless where the loss or the potential for loss is high. In other occasions, a safety manager should involve high-level ranks to the determining or rather to the investigation of a process.

Conclusion
Accidents should not be ignored. They should instead be reported at the time of its occurrence. This will help in the reduction of the number of intentional accidents for fear of the law. Accidents are a part of life but its causes could lead to long time regrets, losses, trauma and sufferings as well.
